Stockton – Patterson Outreach

Join Hart To Hart Ministries, Inc. as we continue our boots-on-the-ground outreach serving individuals and families experiencing homelessness and hardship throughout Stockton, concluding our outreach in Patterson, the community we proudly call home.

 

Our outreach is rooted in building relationships through compassion, dignity, and hope. We believe every conversation matters. Our volunteers take time to listen, encourage, pray with those who desire prayer, share the hope found in Jesus Christ, and remind each individual that they are seen, valued, and loved. We strive to meet both practical and spiritual needs, recognizing that a caring conversation can be just as meaningful as the supplies we provide.

 

Throughout the morning, our outreach team distributes food, bottled water, hygiene supplies, blankets, sleeping bags, clothing, seasonal necessities, care packages, and other essential items based on current needs. We also assist individuals in connecting with community resources that can provide longer-term support.

 

Hart To Hart Ministries is affiliated with the Homeless Management Information System (HMIS) in San Joaquin County, allowing us to help connect individuals with additional services and available resources. As relationships develop, we work alongside organizations such as the Gospel Rescue Mission of Stockton and other local agencies whenever appropriate to help individuals move toward greater stability, housing opportunities, and supportive services.

 

Our commitment extends beyond a single outreach. We continue returning to the community, building trust and strengthening relationships while walking alongside individuals until every reasonable opportunity has been explored to connect them with the care, resources, and support they need.
